Confidential Disclosure Agreements (CDA)

A CDA also known as a Non-Disclosure Agreement (NDA) is a legal contract between at least two parties which outlines confidential materials or knowledge the parties wish to share with one another, but wish to restrict from generalized use.  Before releasing confidential information, please complete and return the CDA Form provided below to our office and we will gladly prepare a Confidentiality/Non-Disclosure Agreement for you.

Material Transfer Agreements (MTA)

A MTA is a contract that governs the transfer of tangible research materials between two parties, when the recipient intends to use it for his or her own research purposes.  Before releasing or requesting materials, please complete and return the MTA Form provided below to our office and we will gladly prepare a Material Transfer Agreement for you.

Sponsored Research Agreements (SRA)

A SRA is an agreement between a private industry party and Catholic Healthcare West for a research project that is directed by a principal investigator.  Please contact our office directly so we may assist you with this process.
